There are so many important transition resources in our communities that we know young people, families and stakeholders need to get connected with in order for transition planning to be successful. But, how do we move from a collection of notes, flyers and business cards to something that everyone can access (and add to)? What if we don’t have all of the info that we need? The answer is virtual community resource mapping! During this session, we’ll cover the core components of a virtual community resource map. We’ll also talk about concrete ways to use them for person-centered planning and travel training. You will get to see plenty of first hand examples for the different ways that virtual maps can be made to support transition planning and instruction.
